Eligibility is determined by several factors including your medical status, health plan coverage and historical medical claims. If you are not initially able to enroll, you may be able to upon meeting eligibility criteria.

⁴ To qualify for a $50 Amazon gift card, members must complete all of the following activities. First, complete enrollment in the new app. Second, complete two consecutive months of engagement within the app. This engagement may include completion of an A1C kit, if clinically indicated, logging of food, messaging your coach, or other in-app activities. Finally, members must complete a short survey at the end of the two months of engagement. To qualify members must be 18 and be eligible with their health plan or employer for Lightpath at the time of survey completion.
