program in the Verily Me app is easy

The Verily Me app is free. The costs of your visits, devices, medication, or supplies may be different for each plan. If you have questions, call the number on your insurance card.
Verily Me is all about giving you control over your health journey, with personalized health recommendations from licensed providers. With that comes the responsibility to protect your records and make sure no one can access your personal information without your consent. If you were an Onduo member, use the same email to enroll in Verily Me. This will ensure your Onduo data is linked and you keep the same care team. For more questions related to migration from Onduo, check out the migration FAQs.

If you were an Onduo member, we want you to have the best care journey. So, we made a more personalized version of your program in a new app called Verily Me that has updated features.2 This includes tools that make it easier and faster to manage your day-to-day health. To switch, click “Enroll now” on this page.
If you are coming from another app or program, your health plan or employer has opted to discontinue the prior program and Lightpath in the Verily Me app is taking its place.
Adults 18 or older with an eligible healthcare plan can enroll. Once enrolled, you’ll answer some questions in the app to determine which program is right for your health needs.
To be eligible you must:
- Be at least 18 years old
- Have been diagnosed with type 1 or type 2 diabetes
- Be covered by an eligible healthcare plan
The Verily virtual care management programs are not intended to support individuals who are pregnant, have an organ transplant history of any kind, have a heart condition requiring a ventricular assist device (e.g., LVAD), or are living with any of the following: severe or end stage kidney failure, cirrhosis or liver failure, dementia (e.g., Alzheimer’s disease), or cystic fibrosis.
To be eligible you must:
- Be at least 18 years old
- Have been diagnosed with high blood pressure/hypertension
- Be covered by an eligible healthcare plan
The Verily virtual care management programs are not intended to support individuals who are pregnant, have an organ transplant history of any kind, have a heart condition requiring a ventricular assist device (e.g., LVAD), or are living with any of the following: severe or end stage kidney failure, cirrhosis or liver failure, dementia (e.g., Alzheimer’s disease), or cystic fibrosis.

- Be interested in losing weight or making healthier lifestyle choices
- Be covered by an eligible healthcare plan

Verily is a separate company that provides health management services for eligible members of BlueAdvantage Administrators of Arkansas, which is an independent licensee of the Blue Cross and Blue Shield Association. Individual eligibility for Verily Lightpath programs varies depending on your employer and/or health plan. If you have any questions about your benefits and costs, check with your benefits administrator or call the phone number on the back of your insurance card.
1 There is no cost for most members. Certain employee benefit plans and health plans, including qualified high deductible plans may result in out of pocket charges for some services and supplies in this solution. To determine if you have any costs for care, you can call the Member Service team at the number on the back of your ID card. If you are eligible, you will receive an invitation to enroll. 4 To qualify for a $50 Amazon gift card, members must complete all of the following activities. First, complete enrollment in the new app. Second, complete two consecutive months of engagement within the app. This engagement may include completion of an A1C kit, if clinically indicated, logging of food, messaging your coach, or other in-app activities. Finally, members must complete a short survey at the end of the two months of engagement. To qualify members must be 18 and be eligible with their health plan or employer for Lightpath at the time of survey completion. Additionally, members must begin migration by 3/31/2026 and complete stated activities by 5/31/2026. There is a limit of one gift card per member.
